Rooming a Cardiology Patient

A clean, consistent rooming workflow that sets up an efficient, safe cardiology visit.

Goal

A consistent rooming routine gets the provider accurate vitals, an updated med list, and a clear reason for the visit — every time.

Step-by-step workflow

  1. 1.Greet, confirm patient identity (name + DOB), and explain what you’ll do.
  2. 2.Confirm the reason for today’s visit in their words.
  3. 3.Take vitals with correct technique (BP, HR, weight, O2 sat as needed).
  4. 4.Review and update the medication list and allergies.
  5. 5.Note any pre-loaded items for this visit type (e.g., weight for HF, EKG if ordered).
  6. 6.Flag any red flags to the nurse/provider; document everything.

What information to collect

  • Chief complaint / reason for visit.
  • Vitals and weight (and trend if relevant).
  • Current medications, adherence concerns, allergies.
  • New or worsening symptoms.

What NOT to do independently

  • Don’t interpret symptoms, change meds, or reassure that something is fine — collect and report.

Scope note

Collect and document; clinical interpretation is the provider’s job.

When to get help

  • Get the nurse/provider immediately for instability — severe dyspnea, fainting, or abnormal vital signs. Report chest pain and get an EKG per protocol.

Interrupt for instability

Stop rooming and get the nurse/provider for an unstable patient. Stable chest pain is reported with an EKG, not an interruption.

Example rooming script

  • "Hi, I’m ___, a medical assistant. Can you confirm your name and date of birth?"
  • "What brings you in today?"
  • "I’m going to take your blood pressure and a few vitals, then go over your medications."

Documentation tips

  • Chief complaint in the patient’s words; vitals with technique notes; med/allergy updates.
