Hypertension Visit Intake

Intake for a BP visit — accurate in-clinic BP, home logs, and adherence notes.

Learning Objectives

  • 1.Obtain an accurate in-clinic blood pressure.
  • 2.Collect home BP logs and adherence information.
  • 3.Flag crisis-range or symptomatic readings.

Goal

Hypertension visits live or die on BP accuracy. Proper technique plus the home log gives the provider what they need to adjust therapy.

What to collect

  • •In-clinic BP with correct technique (rested, correct cuff, arm supported); recheck if high.
  • •Home BP log / readings.
  • •Medication adherence and side effects.
  • •Symptoms: headache, chest pain, vision changes.

Step-by-step

  1. 1.Confirm identity; let the patient rest 5 minutes.
  2. 2.Measure BP correctly; recheck if elevated.
  3. 3.Collect the home log; note adherence; document.

Red flags to report immediately

  • •BP ≥180/120 with symptoms (chest pain, severe headache, vision/neuro changes, dyspnea).

Alert now

A crisis-range BP with symptoms is a red flag — report to the nurse/provider right away.

What NOT to do independently

  • •Don’t adjust BP meds or interpret the readings.

Scope note

Accurate measurement and the log in; treatment decisions are the provider’s.

  • •Record BP (arm/position), rechecks, home log summary, and adherence.
