Learning Objectives
- 1.Collect focused intake for AFib/palpitation visits.
- 2.Acquire an EKG when ordered and check the pulse.
- 3.Recognize red flags to escalate.
Goal
Rhythm visits need a clear symptom picture, a pulse/EKG, and a note on whether the patient is on a blood thinner.
What to collect
- •Palpitations now or intermittent? Fast/slow, regular/irregular?
- •Associated chest pain, lightheadedness, near-fainting, shortness of breath?
- •Heart rate and rhythm (count an irregular pulse for 60 seconds).
- •EKG if ordered; current anticoagulant and adherence.
Step-by-step
- 1.Confirm identity; ask if symptoms are active.
- 2.Take vitals and pulse; acquire EKG if ordered.
- 3.Note anticoagulation; flag red flags; document.
Red flags to report immediately
- •Palpitations with chest pain, fainting/near-fainting, or severe shortness of breath; very fast pulse with symptoms.
Alert now
Rhythm symptoms with chest pain, syncope, or severe dyspnea → alert the nurse/provider immediately.
What NOT to do independently
- •Don’t interpret the rhythm/EKG; collect and acquire.
Scope note
Acquire and report; interpretation is the provider’s.
Documentation tips
- •Note rate/regularity, associated symptoms, EKG, and anticoagulation status.
Checklist
- Asked if symptoms are active; checked pulse.
- Acquired EKG when ordered.
- Noted anticoagulant and adherence.
- Escalated red-flag combinations.
