Hyperlipidemia / ASCVD Prevention

Overview

Lipid management is high-yield prevention. The framework is: identify the risk group, start the right statin intensity, then add agents to reach LDL thresholds in higher-risk patients.

Why this matters

LDL lowering reduces ASCVD events proportionally to the reduction. The biggest gains come from getting high-risk patients on adequate therapy and keeping them on it.

The four statin-benefit groups

  • Clinical ASCVD (secondary prevention) → high-intensity statin.
  • LDL ≥190 mg/dL → high-intensity statin.
  • Diabetes, age 40–75 → at least moderate-intensity (high if higher risk).
  • Primary prevention (40–75, LDL 70–189) → estimate 10-year ASCVD risk and use shared decision-making.

Statin intensity & LDL thresholds

  • High-intensity statins lower LDL ~≥50%; moderate ~30–49%.
  • Very high-risk ASCVD: aim LDL <55 (≥50% reduction); add-on therapy if above threshold.
  • Primary-prevention threshold often LDL ≥70 to intensify in higher-risk patients.

Add-on (non-statin) agents

  • Ezetimibe — first add-on when not at goal on a maximally tolerated statin.
  • PCSK9 inhibitors — for high-risk patients still above threshold (or statin-intolerant).
  • Bempedoic acid — an option in statin intolerance.
  • Very high triglycerides → address (pancreatitis risk); icosapent ethyl in selected high-risk patients.

When CAC helps

  • Coronary artery calcium score helps refine risk in selected intermediate-risk primary-prevention patients when the statin decision is uncertain.
  • CAC of 0 supports deferring statin in selected patients; high CAC supports treating. Not used in acute chest pain.

Monitoring & safety

  • Recheck a lipid panel ~4–12 weeks after start/change to assess response/adherence.
  • Statin muscle symptoms: evaluate, consider dose/agent change — don’t reflexively stop forever.
  • Screen for secondary causes (hypothyroidism, etc.) when appropriate.

Red flags / escalation

  • Very high triglycerides (pancreatitis risk); suspected familial hypercholesterolemia (very high LDL, family history) for specialist input.

Common beginner mistakes

  • Under-dosing statins in secondary prevention.
  • Stopping a statin permanently for minor or non-statin muscle aches.
  • Forgetting to add ezetimibe/PCSK9 when above threshold.
  • Using CAC in the wrong setting (acute chest pain).

Secondary prevention warrants high-intensity statin and an LDL goal <55. Intensify to high-intensity; if still above threshold, add ezetimibe, then consider a PCSK9 inhibitor. 92 is well above target for this very-high-risk patient.

A coronary calcium score can refine the decision: CAC 0 supports deferring, a meaningful score supports starting. It’s a good tie-breaker for intermediate-risk primary prevention.
