Overview
Labs support the story; they don’t replace it. The two classic traps are treating a number in isolation and forgetting the monitoring that prevents medication harm.
Troponin
- •High-sensitivity troponin detects myocardial injury; interpret with the clinical picture and as a trend (serial values / delta).
- •Elevation = injury, not automatically a type 1 MI — many causes raise troponin.
- •A single value rarely tells the whole story; rising/falling patterns matter.
Natriuretic peptides (BNP / NT-proBNP)
- •Support a heart-failure picture; higher values correlate with elevated filling pressures.
- •Confounders: age, renal function, obesity (lower), AFib (higher).
- •Use alongside exam and echo, not alone.
Basic metabolic panel (BMP)
- •Potassium — central to diuretics, ACE/ARB/ARNI, MRAs; both high and low are dangerous.
- •Creatinine/eGFR — drives drug dosing and diuretic response.
- •Sodium — hyponatremia can mark advanced heart failure.
CBC, lipids, TSH, others
- •CBC: anemia worsens angina/HF; bleeding on anticoagulation.
- •Lipids: ASCVD risk and statin management.
- •TSH: thyroid disease causes AFib, tachy/bradycardia, and HF effects.
- •Magnesium (arrhythmia/QT), digoxin level (narrow window) when relevant.
Monitoring that keeps meds safe
- •Starting/up-titrating ACE/ARB/ARNI or MRA → recheck potassium and creatinine.
- •Diuretics → potassium, renal function, volume.
- •Warfarin → INR; digoxin → level and potassium; certain antiarrhythmics → QT/electrolytes.
Red flags / critical values
- •Potassium <3.0 or >6.0 (per lab thresholds) — arrhythmia risk.
- •Acute kidney injury / rapidly rising creatinine.
- •Positive/rising troponin with symptoms; markedly elevated BNP with distress.
Escalate
Critical electrolytes, AKI, or a positive troponin with an ACS picture are time-sensitive — escalate and act promptly; don’t leave them in the inbox.
Common beginner mistakes
- •Treating an isolated troponin/BNP without the clinical context or trend.
- •Starting K-affecting meds without planning the recheck.
- •Ignoring anemia or thyroid disease as drivers of cardiac symptoms.
Documentation pearls
- •Document your interpretation in context and the monitoring plan (what lab, when).
Mini cases
A patient started on lisinopril and spironolactone returns with K 6.2 and a rising creatinine.
What happened and what now?

Hyperkalemia and AKI from combined RAAS/MRA therapy — a critical value. Escalate/act promptly: hold the offending agents per provider guidance, treat hyperkalemia as indicated, and recheck. This is why K/Cr monitoring after starting these meds is mandatory.

Type 1 MI?
